In the majority of cases injuries to the clavicle and the acromion joint occur by:

Falling onto the point of the shoulder causing a sprain to the AC ligament.

Straining the deltoid attachment

Straining of the trapezius attachment

Fractures to the acromial process clavicle and ribs
